PROBLEMS INVOLVING THE ILLEGIBILITY OF MEDICAL PRESCRIPTIONS FOR PHARMACISTS

Abstract

Introduction: The illegibility of medical prescriptions is a serious issue in healthcare, with direct consequences for patient safety and the quality of care. In a context where the increasing demand for medications is combined with the complexity of treatments, the clarity of prescriptions is essential to ensure therapeutic effectiveness and prevent medication errors. Objectives: This study aimed to investigate the issues arising from the illegibility of medical prescriptions from the perspective of pharmaceutical care, identifying the main contributing factors, evaluating the risks associated with misinterpretation, describing the pharmacist’s role in addressing this issue, analyzing the impacts on patient safety, and proposing strategies to minimize these problems Methodology: This is a qualitative, descriptive, and exploratory study conducted through a bibliographic review. Data collection was performed using the Virtual Health Library (VHL), SciELO, and PubMed databases, including publications from 2022 to 2026 in Portuguese and English. Studies available in full text and directly related to the topic were included, while those not meeting the criteria were excluded. Data were analyzed using descriptive and interpretative approaches. Results: The findings indicated that poor prescription legibility is largely associated with improper handwriting, excessive workload, and lack of standardization. It was observed that this issue may lead to medication errors, adverse events, delays in care, and increased healthcare costs. The pharmacist plays a crucial role in identifying inconsistencies, guiding patients, and promoting the rational use of medications. Conclusion: Therefore, the illegibility of medical prescriptions compromises patient safety, making it necessary to invest in measures such as electronic prescribing, standardization of prescriptions, training of healthcare professionals, and a more active role of the pharmacist. These actions are essential to ensure greater safety, efficiency, and quality in healthcare delivery.
