Chronic rhinosinusitis with nasal polyps is a persistent inflammatory disease of the upper airways associated with a significant impact on quality of life, frequent clinical recurrence, and high respiratory morbidity. In recent years, advances in the understanding of type 2 inflammation have enabled the development of biologics targeting specific mediators involved in the pathophysiology of the disease, making these therapies promising alternatives for patients refractory to conventional treatment. The present study aimed to analyze the role of biologics in the treatment of chronic rhinosinusitis with nasal polyps, evaluating their clinical efficacy, impact on quality of life, and safety profile. This is a systematic literature review conducted according to PRISMA 2020 recommendations, with searches performed in the PubMed/MEDLINE, Virtual Health Library, and PubMed Central databases. Studies published between 2016 and 2025 involving adult patients treated with biologics were included. The results demonstrated that dupilumab and omalizumab promoted significant reduction of nasal polyps, improvement in nasal congestion, partial recovery of olfaction, and improvement in quality of life, in addition to presenting a favorable safety profile. It is concluded that biologics represent an important therapeutic alternative for patients with severe or refractory chronic rhinosinusitis with nasal polyps, contributing to better inflammatory control and significant clinical improvement.
