Introduction: Dengue is consolidated as a severe public health problem in Brazil, characterized by high morbidity and cyclical regional behavior. Objective: To elaborate the epidemiological profile of dengue in Alagoas between the years 2021 and 2025. Methods: This is an ecological, descriptive, and retrospective study structured from secondary data of compulsory notifications registered in the SINAN/DATASUS platform. Results: A total of 71,647 probable cases were registered, with marked epidemic peaks in 2022 and 2024. The 1st Health Region concentrated 54.33% of all occurrences. There was a predominance of infections in females (54.63%), young adults aged 20 to 39 years (41.39%), and self-declared brown individuals (65.04%). Although the vast majority of patients evolved to cure, 51 deaths from the disease were confirmed. Furthermore, significant laboratory underreporting was identified, with over 90% of tests classified as ignored or not performed. Conclusion: Dengue primarily affects the economically active and vulnerable population in urbanized centers. Chronic registration failures indicate structural weaknesses in the system, highlighting the urgent need to strengthen health surveillance and intersectoral vector control policies.
