The periapical cyst is the most prevalent odontogenic inflammatory lesion in clinical practice, associated with teeth with pulp necrosis. In immunosuppressed patients due to conditions such as HIV, diabetes, cancer, or the use of immunosuppressive medications, the diagnosis and treatment of these lesions present significant challenges. Immunosuppression impairs the inflammatory response and tissue repair capacity, increasing the risk of complications and negatively influencing the prognosis. This literature review aims to consolidate current knowledge on the particularities of the periapical cyst in this population, discussing the best therapeutic strategies. It is concluded that management requires detailed planning, considering comorbidities, with an emphasis on infection control and the adaptation of protocols, although gaps in the literature persist, demanding further studies to establish standardized and effective conducts.
