CLINICAL PHARMACY AND PHARMACEUTICAL CARE IN THE HOSPITAL ENVIRONMENT

Although previously limited to the storage and distribution of medications, hospital pharmacy has evolved with the introduction of pharmaceutical care, allowing greater control over the quality and safety of medication use. This study aimed, through a literature review, to highlight the presence of clinical pharmacy and pharmaceutical care in the hospital environment, as well as to understand the role and importance of pharmacists in this setting. The research is exploratory and descriptive, using a systematic literature review. Articles were selected through searches in Google Scholar, PubMed, LILACS, and Scielo. The results revealed that the role of the clinical pharmacist is adaptable to different contexts. In this regard, pharmacists perform pharmacotherapeutic monitoring, contributing to the reduction of errors, costs, and risks related to medication use, in addition to improving treatment efficacy through personalized consultations and interventions. In conclusion, the clinical pharmacist is essential in the hospital setting, ensuring optimal drug therapy and helping to prevent medication-related problems. Multidisciplinary collaboration and the acceptance of pharmaceutical interventions by prescribers are crucial for improving patient adherence to treatment and promoting health.
