This article aimed to analyze the role of cone-beam computed tomography (CBCT) in the navigation of obliterated root canals, with emphasis on reducing the risk of perforation and on preserving tooth structure. It is an integrative literature review that included 23 studies published between 2021 and 2025, comprising reviews, case reports and case series, laboratory studies, one cross-sectional study, an expert consensus and clinical guidance. The results were organized into five axes: canal location and access, risk of perforation and deviation, preservation of tooth structure, static and dynamic navigation, and limitations of CBCT. The studies described the location of calcified canals through accesses planned with CBCT and guides, with few accidents reported; however, the evidence consists predominantly of case reports and in vitro studies. Guided techniques have restrictions in posterior teeth and curved roots. It is concluded that guided endodontics is a promising alternative, but CBCT requires careful indication, and clinical trials with standardized protocols are needed.
