The implementation of Lean principles in the operating room has emerged as an effective strategy to enhance operational efficiency, reduce waste, and strengthen patient safety. This integrative review analyzed 17 studies published between 2015 and 2025, identified in the MEDLINE, LILACS, BDENF, and IBECS databases. Findings indicate that Lean interventions contribute to reducing delays, optimizing operating room utilization, standardizing workflows, and decreasing variability among teams, promoting greater predictability and procedural flow. Lean practices also improve interprofessional communication and reduce intraoperative disruptions, which are critical for patient safety. Sustainability aspects were also identified, demonstrating reduced waste generation and more rational material use. Despite the benefits, challenges persist, including team resistance, technological limitations, and insufficient continuous training.
