Introduction: Cleft lip and palate is a congenital malformation with a multifactorial etiology that compromises facial structures, oral functions, and psychosocial aspects of the individual. This condition can result in significant changes in craniofacial growth, often associated with Class III malocclusion, difficulties in speech, chewing, and social interaction. Objective: This study aims to analyze the orthodontic clinical management of a patient with cleft lip and palate, highlighting the therapeutic stages and the challenges of interdisciplinary management. Methodology: This is a clinical case report of a 12-year-old male patient diagnosed with complete unilateral cleft lip and palate, followed up in a specialization course in Orthodontia. After clinical evaluation and analysis of complementary exams, an orthodontic treatment plan was instituted using a Hyrax-type maxillary expander combined with a Petit facial mask, aiming at maxillary expansion and protraction. The treatment was conducted over 12 months, with continuous monitoring and progressive adjustments of orthopedic forces. Results: Significant improvement was observed in the maxillomandibular relationship, dental alignment, and facial aesthetics of the patient, in addition to space gain for tooth eruption. Conclusion: It is concluded that early orthodontic intervention, combined with a multidisciplinary approach, is essential for the functional, aesthetic, and psychosocial rehabilitation of patients with cleft lip and palate, contributing to an improved quality of life.
