Ensuring access to and quality of health care for people experiencing homelessness (PEH) remains a challenge for the public health system, perpetuating inequities and social exclusion. From this perspective, this study aimed to report the experience of intersectoral actions between social assistance and health services for PEH in a municipality in Minas Gerais, Brazil, highlighting the strategies, challenges, and outcomes achieved. This is an experience report developed between 2023 and 2024 through coordination between Primary Health Care and the Unified Social Assistance System, involving joint planning, field-based outreach, and systematization of records. A total of 37 field interventions were conducted, resulting in 47 health care encounters, including clinical procedures in nursing and dentistry, psychological support, and social assistance. The actions proved effective, particularly during periods of cold weather, with relevant acceptance among the population. This experience reaffirmed the need for intersectoral coordination to ensure universal rights and comprehensive care. Limitations in the systematization of records represented important challenges.
