Diastasis of the rectus abdominis muscles (DRAM) is characterized by separation of the rectus muscles along the linea alba and may be associated with changes in abdominal wall function and stability. The Pilates method has been used as a physiotherapeutic resource because it involves exercises aimed at strengthening, controlling, and stabilizing the trunk musculature. This study aimed to analyze the evidence on the use of Pilates in the rehabilitation of DRAM. This systematic literature review was conducted using PubMed, SciELO, LILACS, PEDro, and Google Scholar. Primary studies published between 2015 and 2025 in Portuguese, English, and Spanish, directly related to Pilates and abdominal diastasis, were included. After identification, screening, and eligibility assessment, four studies were included. Three studies observed a reduction in inter-recti distance, whereas one study conducted during pregnancy did not identify a reduction in diastasis. Improvements in abdominal muscle strength and endurance and a reduction in low back pain were also observed. The findings indicate that Pilates was effective in reducing inter-recti distance in most studies and may be considered a physiotherapeutic resource applicable to the management of DRAM, although study heterogeneity limits the generalization of the results.
