This article aimed to analyze the use of platelet-rich fibrin (PRF) in the prevention and treatment of Medication-Related Osteonecrosis of the Jaws (MRONJ), especially in patients undergoing treatment with antiresorptive and antiangiogenic drugs. This is a narrative literature review carried out through searches in the PubMed/MEDLINE and SciELO databases, using scientific articles published between 2021 and 2026. Studies related to the clinical application of PRF in the management of MRONJ were included, considering its different variations, such as leukocyte- and platelet-rich fibrin (L-PRF), advanced platelet-rich fibrin (A-PRF), and injectable platelet-rich fibrin (i-PRF). The analyzed studies demonstrated that PRF presents relevant regenerative potential, favoring angiogenesis, tissue healing, and bone regeneration, in addition to contributing to better clinical resolution rates when associated with conventional surgical treatment. However, methodological heterogeneity was observed among the studies, mainly regarding PRF preparation and application protocols. It is concluded that PRF presents promising therapeutic potential as a complementary alternative in the prevention and treatment of MRONJ, although further standardized clinical studies are still necessary to consolidate its clinical applicability.
