Clinical deterioration in hospitalized patients represents a significant challenge to patient safety, potentially resulting in severe complications, the need for intensive care, and increased mortality. Early recognition of physiological changes is a fundamental strategy for preventing adverse outcomes and facilitating timely interventions. This article aims to discuss, through a narrative literature review, the main challenges associated with identifying clinical deterioration and the strategies used to improve hospital care. Particular emphasis is placed on systematic monitoring of vital signs, the use of early warning scores, individualized clinical assessment, and effective communication among healthcare professionals. Furthermore, structural limitations, healthcare team workload, and difficulties in implementing clinical protocols are addressed. The integration of assessment tools, professional training, and organized care pathways has the potential to strengthen patient safety. It is concluded that timely recognition of clinical deterioration depends on a systematic, multidisciplinary approach grounded in scientific evidence.
