This study aimed to analyze the epidemiological profile, temporal trend, and hospital outcomes of hospitalizations for complications of Diabetes Mellitus in the state of Paraná, Brazil, from 2015 to 2025. This was an epidemiological, observational, descriptive, quantitative study based on secondary data, with an ecological design and time-series analysis, using data from the Hospital Information System of the Brazilian Unified Health System (SIH/SUS), made available by DATASUS. A total of 81,346 hospitalizations were recorded, with an annual average of 7,395 hospitalizations, with no significant temporal trend (β = −37.9; 95% CI: −115.0 to 39.4; p = 0.296; R² = 0.120). Individuals aged 50 years or older predominated (68.0%), with a slight predominance of females and a higher frequency of individuals who self-identified as White. A total of 2,181 in-hospital deaths were recorded, while the Eastern Macroregion had the highest concentration of hospitalizations. An approximately 62% increase in total hospitalization expenditures and a 60.6% increase in the mean expenditure per hospitalization were observed between 2015 and 2025. In conclusion, hospitalizations remained high, with territorial disparities, in-hospital deaths, and increasing hospitalization expenditures, highlighting the need for actions aimed at preventing complications and reducing potentially avoidable hospitalizations.
