Introduction: Diabetes mellitus (DM) has a high prevalence and can lead to chronic kidney disease (CKD), a major cause of morbidity. Objective: To understand the knowledge held by people with DM regarding its relationship with CKD in a municipality in the Agreste region of Pernambuco. Methodology: A descriptive, exploratory field study using a mixed-methods approach, conducted with 14 individuals with type 2 diabetes (T2DM) registered at a Basic Health Unit. Semi-structured interviews were used, featuring closed-ended questions to characterize the participants and open-ended questions to identify their knowledge and perceptions. Results: Most participants were women (78.6%) and individuals with low levels of education (71.4%). The duration since diagnosis was not associated with greater knowledge regarding CKD. Although participants recognized complications such as neuropathy, hypertension, vision loss, and amputations, they demonstrated limited understanding of the causes and prevention of CKD, often limiting their knowledge to fluid intake. Care was primarily provided within the Primary Health Care system, revealing weaknesses in patient education and the continuity of care. Conclusion: Gaps in knowledge regarding the relationship between DM and CKD were identified, highlighting the need for continuous, culturally appropriate educational initiatives integrated into Primary Health Care to strengthen self-care and prevent renal complications.
