The electronic health record (EHR) in Primary Health Care (PHC) is one of the most significant digital transformation initiatives within the Unified Health System (SUS), aimed at improving care delivery, enhancing patient safety, and optimizing service management. This study aimed to evaluate the effects of adopting EHRs in PHC, particularly regarding the quality of care, patient safety, and the efficiency of health management. This is an integrative literature review conducted using the PubMed, Virtual Health Library (VHL/BVS), Scientific Electronic Library Online (SciELO), and CAPES Journal Portal databases, covering publications from 2021 to 2025. Five studies meeting the eligibility criteria were selected; their findings indicated that EHRs support continuity of care, improve the organization of clinical information, strengthen evidence-based decision-making, and enhance management processes. However, obstacles remain regarding system interoperability, technological infrastructure, professional training, and information governance. The EHR is a strategic tool for the modernization of PHC, requiring ongoing investment in technology, professional training, and system integration.
