Abstract

This study comparatively analyzed the clinical and neurosurgical outcome of traumatic subdural hematomas (HSDTs), a serious complication of traumatic brain injuries. An integrative literature review was conducted to identify studies that directly compared clinical and neurosurgical therapeutic approaches for HSDTs. A total of 15 studies were included in the analysis, covering randomized controlled trials and prospective observational studies. The results showed that neurosurgical treatment of HSDTs was associated with a significant reduction in mortality and improved functional recovery compared with conservative medical treatment. However, surgical complications, such as surgical site infection and recurrence of subdural hematoma, were observed in a significant proportion of patients undergoing surgical intervention. Comparative analysis of surgical approaches also highlighted the lack of consensus on the most effective surgical technique for managing HSDTs. Although some studies have suggested benefits of decompressive craniotomy, others have shown favorable results with minimally invasive subdural drainage. In conclusion, neurosurgical treatment emerges as an effective therapeutic option for patients with HSDTs, especially in severe and symptomatic cases. However, careful patient selection and the execution of appropriate surgical techniques are essential to minimize risks and maximize results. Further studies are needed to determine the optimal surgical approach and guide clinical practice in this constantly evolving area.
