This experience report aimed to describe the physical therapy care combined with the Pilates method for the functional rehabilitation of two individuals with lumbar disc extrusion who underwent different management strategies. It is a descriptive and reflective report based on the physical therapy monitoring of two distinct cases: one following microdiscectomy and the other following epidural injection. Data were systematized regarding clinical manifestations, functional limitations, therapeutic strategies, session frequency, functional progress, and insights gained from the practice. In the post-surgical case, a significant reduction in pain was observed after the procedure, followed by improvements in balance, muscle tone, and functional control through progressive rehabilitation. In the conservative management case, pain reduction following the injection facilitated the continuation of physical therapy, leading to progressive improvements in gait, muscle strength, and sensory symptoms. In both cases, the Pilates method was applied in an individualized manner and combined with other physical therapy modalities. The experience reinforced the importance of tailoring interventions, continuous assessment, and an interdisciplinary approach to the rehabilitation of lumbar disc extrusion, taking into account each individual's specific needs and limitations.
