Diastemas and morphological alterations in anterior teeth may compromise smile harmony and motivate patients to seek aesthetic treatment. This study reports the aesthetic reanatomization of anterior teeth with composite resin in a patient presenting a midline diastema, a peg-shaped lateral incisor, and reduced anatomy of the maxillary canines. After clinical evaluation, planning included impressions, diagnostic waxing, and fabrication of a silicone guide. The patient previously underwent in-office dental bleaching. A modified rubber dam isolation was then performed, followed by acid etching, application of the adhesive system, and composite resin layering with the aid of the silicone guide. After finishing, occlusal adjustment, and polishing, diastema closure and improvement in the shape and proportion of the anterior teeth were observed, resulting in immediate smile harmonization. The direct composite resin technique proved to be a conservative and relatively fast clinical alternative for the aesthetic correction of the presented case.
